Media Consent & Release Form
I hereby give Alley Kat Brewing Co. and its authorized representatives permission to photograph, film, record, and/or interview me and to use content featuring my image, likeness, voice, statements, and/or appearance for marketing, promotional, communications, and related business purposes.
Permission & Use
I understand and agree that Alley Kat Brewing Co. may:

Photograph, film, video record, or audio record me.

Record interviews, testimonials, comments, or other statements I voluntarily provide.

Use my name, image, likeness, voice, and/or statements in connection with the resulting content.

Edit, crop, reproduce, publish, distribute, and otherwise use this content in print and digital formats.

Use the content on social media platforms, websites, digital advertising, email marketing, promotional materials, press/media communications, presentations, signage, and other marketing channels.

Use the content without requiring additional approval from me before publication.
Compensation
I understand that, unless otherwise agreed to in writing, my participation is voluntary and I will not receive payment, royalties, or other compensation for the creation or use of this content.
Release
To the extent permitted by law, I release Alley Kat Brewing Co., its employees, representatives, contractors, and authorized partners from claims arising from the authorized use of my image, likeness, voice, or statements under this release.
I understand that content published online or through social media may be shared, reposted, or otherwise distributed by third parties outside of Alley Kat Brewing Co.'s control.
By signing below, I confirm that I have read and understood this Media Consent & Release Form and voluntarily agree to its terms.
